The Generic Count sheet has been developed by ACORN to guide Healthcare Facilities (HCF) in formatting a document to match their perioperative needs.

The Generic Count sheet has been developed by ACORN to guide Healthcare Facilities (HCF) in formatting a document to match their perioperative needs.  As a generic document ACORN would advise HCF’s to adapt it as required for their unit.  The aim of this document is for HCFs to use generic terminology and to give guidance on what should be documented on the actual count sheet.  All other perioperative records pertaining to patient care should be maintained.

In developing this Generic Count sheet ACORN requested copies of count sheets from all states and territories; the information collated has resulted in the information that has been utilised in the document.

Count Items:  These are generic terms used in the ACORN Standard S3 Counting of Accountable Items Used During Surgery. HCF’s would be encouraged to maintain correct terminology but add specific items related to their specialties.

Document Orientation:  ACORN have developed the document in portrait but it can be changed to landscape if it suits HCF documentation.

Countable items remaining in situ:  If these items are documented on the perioperative approved documentation this section can be removed.
